The USDA, just one of the agencies investigating the fatal tiger attack on zookeeper Stacey Konwiser on April 15at the Palm Beach Zoo, released a one page report Wednesday on the incident.
The USDA said Konwiser "did not follow established safety procedures."
The report says the need to dart and sedate the tiger subjected the animal to behavioral stress, unnecessary discomfort and potential harm which could have been prevented if adequate safety protocols were followed.
The report also raises concern about maintenance near the tiger enclosure, saying there were several piles of debris and tree limb cuttings that could cause a zookeeper to fall.
