A free immunization clinic will be held at the Palm Beach County School District welcome center Thursday from 10 a.m. until 1 p.m.
It's being conducted by the Florida Department of Health.
The location is 3300 Forest Hill Blvd., Building No. 4 in West Palm Beach.
Children who 2-months-old through 12th grade need to have their immunization records.
You will need to bring:
- Two current documents reflecting the correct residential street address
- Florida certification of immunization
- State of Florida School Entry Health Exam
- Proof of birth (birth certificate, passport, religious records)
