Buckle Up Kids: Contest Rules

TO ENTER:

Beginning May 28, 2012, on a 3x5 card, hand print your complete name, street address (no P.O. Boxes), city, state, zip code, email address, daytime and evening telephone numbers. (If you do not have a telephone, state “No Phone”.)  Failure to comply with this or any other official rule will result in disqualification).  Mail your entry in a hand-addressed, business-size envelope (#10) with first-class postage, to: Buckle Up Kids Booster Car Seat Give Away, 1100 Banyan Blvd., West Palm Beach, FL  33401. Entries must be received by July 2, 2012 by 5:00 PM EST. Limit one entry per envelope.

PRIZES:

(20) First Prizes - Winners will each receive a child’s booster car seat.  Brand of booster car seat will be determined by the Safe Kids Coalition of Palm Beach County.  Winners will be contacted by the Safe Kids Coalition to arrange a time for winner to come to the Safe Kids Coalition offices located at 2001 W Blue Heron Blvd., Riviera Beach, Florida 33404, to pick up their booster car seat.  Each winner will receive proper booster car seat installation and usage training; and sign an affidavit stating winner has received proper instruction.
